The Milford School District Nursing Department has partnered with Rite Aid Pharmacy to sponsor a Student Covid Vaccination Clinic. Please read the following in its entirety, as there is a lot of information to ensure the clinic goes smoothly. Time and location The first vaccination clinic will be held on Thursday, December 2, 2021, from 4 p.m. until 7 p.m. The clinic will be held at Jacques Memorial Elementary School located at 9 Elm Street, in the Cafeteria. The second vaccination clinic will be held on Thursday, January 6, 2022, from 4 p.m. until 7 p.m at Jacques Memorial Elementary School. The link for sign up for the second clinic will be sent after the first clinic is completed. Please keep in mind that this is for Milford School District students only. This clinic is completely voluntary and your child is NOT required to participate. Please check with your child’s Health Care Provider for questions regarding the vaccine. ****Milford School District does not keep a record of students attending the clinic. If you want the immunization to be a part of your child’s medical record, you may supply the school nurse with a copy.**** This clinic will only have the Pfizer vaccine available for both age groups 5-11 and 12 and up.
